Health Service

The North West Maternity Services (NWMS) is primarily located at the North West Regional Hospital, with satellite and community-based services delivered across Mersey Community Hospital (MCH), King Island District Hospital, Smithton District Hospital and West Coast District Hospital. NWMS covers the full scope of maternity care including antenatal clinics, parenting education classes, pregnancy and postnatal assessment services, a 4-bed birthing suite, 20 bed inpatient ward, 8 bed special care nursery and extended care midwifery services for women when they discharge home after birth. NWMS also offers women dedicated lactation services as well as connections through allied health colleagues for social work, women’s health physio and diabetes services and perinatal mental health.

What you'll be doing

The aim of SUPPS is to provide coordinated, collaborative and client centered care to people who use substances and are pregnant and/or the parents of children under the age of 2. SUPPS takes a harm minimisation, trauma informed and client centered approach to care to promote the social, emotional and physical wellbeing of women, children and families. SUPPS also aims to build the capacity of other health services to identify, respond and engage people in care. The SUPPS social worker will provide a specialist social work service including intensive case management, counselling and home visits. The role works alongside the SUPPS Clinical Nurse/Midwife Specialist and within the wider Alcohol and Other Drugs (AOD) team.

Selection Criteria

• Tertiary qualifications in social work and eligibility for membership with the Australian Association of Social Workers (AASW)
• Extensive post-graduate clinical experience in alcohol and other drugs counselling, child protection, domestic violence, or family relationship issues, with a strong commitment to partnering with Aboriginal clients and improving health outcomes for socially disadvantaged groups
• Proven experience in using advanced reasoning skills to prepare high-quality written reports, oral presentations, and comprehensive care plans, along with strong time management abilities
• Demonstrated leadership in quality improvement initiatives, service evaluation, and clinical supervision of staff
• Demonstrated understanding of the principles of trauma informed care and its application to interventions with pregnant people and their families
• Demonstrated proficient written, verbal and interpersonal communication skills
• Demonstrated skills in the use of computer based clinical record keeping and other practice related software

About Ravenhall Correctional Centre

Ravenhall is a 1,300-bed medium-security prison for adult sentenced and remand men. Its contemporary campus-style design reflects four accommodation communities with varying profiles. Community accommodation is complemented by a further 75-bed Forensic Mental Health Unit, a 25-bed unit for prisoners with significant challenging behaviours, and a 25-bed Mission Unit designed to respond intensively and therapeutically to current community issues.

About The Opportunity

The Family and Community Connection Manager is accountable for the effective delivery, quality and continuous improvement of evidence-informed services and programs that strengthen family reconnection, cultural and faith identity, and youth wellbeing for men in custody. The role supports the planned coordination and continuity of these services in the community following release, in partnership with the Community Reintegration Centre.

Reporting to the Rehabilitative Services Manager, the role translates strategic, contractual and evidence-based priorities into coordinated operational delivery. It leads multidisciplinary staff and works with custodial, clinical, rehabilitation, reintegration and community partners to provide culturally responsive, person-centred supports that reduce barriers to rehabilitation and strengthen successful reintegration.

As the Family and Community Connection Manager you will:
