Grampians Health in Ballarat is seeking a geriatric medicine specialist to provide comprehensive care across hospital, community, and ambulatory settings. You will deliver consultative and shared-care services in acute and subacute hospital environments, supervise ward teams, and support Hospital in the Home programs.

Active involvement in professional development, research, and education is encouraged, with participation in the Victorian Geriatric Medicine Training Program and cross-service

• Provide geriatric expertise across hospital, community, and ambulatory settings
• Deliver consultative, shared-care, and primary geriatric care in acute and subacute hospital settings
• Supervise and coordinate patient care within subacute wards and teams
• Support admitted services including Hospital in the Home and GEM at Home
• Contribute to non-admitted services including the Hospital Admission Risk Program, Residential In-Reach, and Transition Care Program
• Conduct or support complex and specialist geriatric medicine clinics, home visits, and remote telehealth
• Attend and contribute to the Victorian Geriatric Medicine Training Program
• Contribute to service development, research, education, and the overall performance of the Geriatric Medicine team
• Align work with the Mission, Vision, and Objectives of Grampians Health

• Registration with the Australian Health Practitioner Regulation Agency (APHRA) is mandatory
• Registered or eligible to register for Advanced Training in Geriatric Medicine with the Royal Australasian College of Physicians
• Appropriate level of experience, knowledge, skills, and aptitude in the clinical practice of medicine
• Physician trainee planning to undertake or currently undertaking advanced training in Geriatric Medicine

Demonstrates expertise in geriatric medicine across various settings, including acute and subacute care, while effectively supervising patient care and contributing to service development and education. Holds registration with APHRA and is eligible for advanced training in Geriatric Medicine.
