Job Description

Reporting to the Care Community Lead, this role will see you:

• Conducting in-home assessments and reviews to guide care planning
• Coordinating your own calendar and managing daily visits across the community
• Supporting person-centered care focused on independence and re-enablement
• Providing sound clinical advice and guidance on complex or escalated care needs
• Collaborating with internal teams, providers and health professionals when required

About the Candidate

We're looking for a confident and experienced clinician with a minimum of two years working autonomously in the field to take on this role that will work with clients in the community while staying connected to a supportive, multidisciplinary team.

You will enjoy getting results for your clients as a result of your ability to conduct thorough assessments and applying sound clinical reasoning skills, which in turn will help older people maintain independence and quality of life at home.

You're someone who:

• Builds trust quickly with customers, families and stakeholders
• Is confident working independently and making decisions in complex or changing situations
• Has proven experience managing a varied case load of clients along with the associated workload
• Communicates with clarity, empathy and professionalism,
• Tertiary qualification in Nursing, Allied Health or related field
• Current AHPRA registration (where applicable)
• Experience in community aged care or complex care settings
• Strong assessment, care planning and clinical decision-making skills
• Ability to work autonomously and manage competing priorities

AnglicareSA is seeking a Clinical Coordinator to provide clinical leadership and education to our care and clinical teams across Grange and Westbourne Park RAC sites. You will mentor staff and ensure delivery of safe, person-centred care in line with contemporary nursing practice.

You will lead care planning, governance and quality improvement, working closely with the Clinical Manager, residents and families to achieve positive outcomes. Strong communication and mentoring are essential.
