As a Care Manager, you'll collaborate closely with the Centre Manager, ensuring clinical governance and overseeing the multidisciplinary care team.

About the role

This role forms part of the Care Finders program, supporting vulnerable clients across Launceston to navigate complex systems and improve their wellbeing. This is a part-time role, located in New Town, Tasmania.

How you will make an impactProvide high-quality, person-centred support to older people and those at risk of homelessness who need help to access aged care and other services.

Help clients connect with My Aged Care and other community supports to improve their wellbeing and independence. Support clients to find safe, sustainable housing and access the care they need to stay well and housed.

Work closely with the wider Care Finders team, offering advice and support with more complex client needs.

Keep clear records and share regular updates and reports with the Care Finders TeamWhat you will bringTertiary qualifications in Social Work, AOD Counselling, Welfare, Psychology, or a related discipline, or substantial relevant experience.

Extensive knowledge of and experience in the housing and homelessness sector in Tasmania.

Demonstrated experience in client-centred case management within housing, homelessness, or related community services settings.

An understanding of relevant Tasmanian and national legislation, policies, and frameworks relating to homelessness, housing, and social support services.

A current Driver's Licence.

A working knowledge of client management systems and Microsoft 365 in a professional setting.

A current Working with Vulnerable People Registration (Tasmania).

We are seeking a values-driven AOD Team Leader to support the day-to-day operations and lead the delivery of participant-led, recovery-focused services. You will provide guidance to the team and ensure high-quality case management, assessment, referral, education and support are delivered in line with TSA guidelines.

Located in Fountaindale on the NSW Central Coast, this full-time, permanent role is based within a 40-bed residential AOD facility and reports to the Service Manager.

Salary: In accordance with SCHADS Award Level 6.

• Report to service manager on service performance with the aim of growing the existing service and client base
• Manage day to day operations, roll out of program changes and service/staff performance
• Support the efficient day to day service delivery through education, interventions, group work, assessment, referral and case work within a recovery orientated and person led service environment
• Monitor the performance of and provide support to direct reports to deliver quality, efficient and effective services
• Build strong referral pathways with internal and external stakeholders, resulting in beneficial outcomes for participants

• Relevant degree or Diploma in AOD, Community Services, Case Management or a related discipline.
• Completion of 4 AOD Skill Set units, or willingness to complete them.
• 3+ years' management experience in AOD, community services, health or related sectors.
• Experience working with criminal justice clients and understanding of the criminal justice system.
• Proven ability to lead teams, build capability and improve service delivery.
• Experience supporting people impacted by AOD, gambling and mental health challenges.
• Experience working with diverse cultural, linguistic and religious backgrounds.
• Strong de-escalation and problem-solving skills when managing challenging or unpredictable situations.
• Willingness to undergo a National Police Check.
