Opportunity available for a 2-year fixed term contract. As System and Service Improvement General Manager - MATSICHS, your role is to provide strategic leadership to strengthen the quality, effectiveness, accessibility and sustainability of services across the organisation. Working within MATSICHS, the role will lead a system and service improvement agenda that places community voice, self-determination, cultural safety, clinical quality and service effectiveness at the centre of system design and decision making.

• Identify opportunities to improve the way services are designed, connected and delivered, working collaboratively with community, clinical teams, operational leaders and external partners to translate evidence, data and lived experience into practical and sustainable improvements.
• Foster a culture of openness, transparency, self reflection and integrity across MATSICHS, encouraging gaps, issues and incidents to be readily identified and acted upon in the interests of continually improving the safety and quality of service provision for clients and community.
• Provide leadership in the development, implementation and evaluation of plans, programs, models and policies for improving Aboriginal and Torres Strait Islander Health and Well being.
• Assess service gaps, community needs and emerging priorities, translating these insights into high quality funding, partnership and investment proposals that strengthen services and improve outcomes for community.
• Provide timely and comprehensive reports for purposes of compliance against funding criteria as well as for purposes of monitoring, feedback and internal communication of key information, and continuous service improvement.

• Relevant tertiary qualification in health, business, quality improvement, service design, public health, management or a related discipline, or equivalent experience.

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region. The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. As General Practitioner - Pamela Mam Health Centre, your role is responsible for the provision of General Practitioner services to Aboriginal and Torres Strait Islander clients and families as part of a multi-disciplinary team of medical and health staff, within the operational framework of an Aboriginal and Torres Strait Islander community-controlled Health Service.

• Contribute to the maintenance of high-quality information in electronic record systems.
• Utilise patient information and recall systems to support individual client care and identify population health needs and gaps in service delivery.
• Establish and maintain strong linkages with other health providers in the area to ensure optimum coordination of care and advocacy for the client base.
• Oversee the delivery of comprehensive preventive health assessments for clients across the age spectrum and provide leadership for the multidisciplinary team in delivery of coordinated care planning for clients with complex chronic care needs.
• Optimise referral pathways to specialist and allied health services for clients; work closely with the Practice Manager and service team to anticipate and identify opportunities for expanding access to these services for clients.
