The Aged Care Single Assessment System (SAS) has been set up by the Health Department to simplify and improve access to aged care services. SAS includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments.

Working as part of the SAS team, and reporting to the SAS Clinical Team Leader, you will be responsible for:

• Undertaking client assessments, reviews and development of support plans to assist clients in achieving their goals and living independently in their own homes and community across all Northwestern Melbourne LGAs.
• Participating in clinical triage and case conferencing
• Identifying, monitoring and managing risks and leading appropriate escalation.

Bring your great, everyday!

To be successful in this role, you will bring demonstrated experience delivering services in aged care settings and/or directly supporting older people, either in Australia or overseas.

You will also bring:

• A tertiary qualification in a health-related discipline directly related to health, aged care, or a specialist area such as Allied Health, Social Work, or Registered Nursing.
• Current unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA) or another relevant professional association.

The fine print

• We will be screening, shortlisting, and interviewing applications as received, so don't delay.
• Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.
• Full driver license and travel between sites required. Travel will also be required to conduct assessments within all Northwestern Melbourne Local Government Areas (LGAs).
