What You Will Be Doing There is an opportunity in the Social Work, Music Therapy, & Art Therapy team at Peter Mac to welcome multiple Grade 2 Social Workers with both full time and part-time permanent positions available. - Responsible for the completion of discharge summaries and coding. HMO roles may be able to be structured to individual interests and O&G experience with junior and senior O&G HMO positions available. About the role As Care Coordinator - Staying Deadly Hub, your role is to provide outcome focused health, social and practical support to Aboriginal and Torres Strait Islander clients. The TeamThe Mental Health Inpatient Unit (MHIU) is located in the Royal Hobart Hospital, level 2 and 3K West. MHIU sits under the Mental Health Department (MHD) within the Statewide Mental Health Services' Acute Care Stream and supports adults with serious mental illness across multiple locations, serving southern Tasmania and those best suited to its services.
The RoleThe Clinical Nurse Specialist - Discharge Planner plays a vital role in supporting safe, timely, and coordinated discharges within a high-demand acute mental health setting. As an integral member of the Multidisciplinary Team, the CNS works collaboratively with treating teams, clinicians, and consumers to ensure person-centred, recovery-oriented discharge planning. The role involves building strong relationships across the broader service network, including NDIS providers and accommodation services, to facilitate smooth transitions to the least restrictive and most appropriate care environment.
You will need- Demonstrated ability to deliver high-quality mental health care in an acute inpatient mental health setting with a particular focus on discharge planning assessments, risk assessments, brief psychological interventions and developing management plans.
- Extensive demonstrated experience working in an inpatient mental health setting in an autonomous role and as part of a multidisciplinary team.
- Demonstrated knowledge of the psychological and healthcare needs of individuals, including the understanding of recovery principles, policies and procedures, and associated legislation which impact upon inpatient and community-based settings.

About the role
We are looking for a passionate and experienced Service Manager to lead the Adult Step-Up Step-Down (ASUSD) service in Gladstone.
In this leadership role, you will lead the team and oversee all aspects of service delivery, ensuring high-quality, recovery-oriented services are provided to people, families and carers in line with Mind's Model of Recovery Oriented Practice and organisational values.
You will be accountable for service performance, budget targets and operational requirements, while building strong relationships with clinical partners, community organisations, government agencies and other key stakeholders.
Key responsibilities
- Lead and manage high-quality, recovery-oriented services for people, families and carers in line with Mind's Model of Recovery-Oriented Practice.
- Provide leadership, coaching and supervision to Team Leaders and multidisciplinary teams to achieve quality service and performance outcomes.
- Oversee day-to-day service operations, including intake, assessment, recovery planning, care reviews, staffing, rostering and service requirements.
- Manage service performance, budgets, reporting, contractual obligations and compliance with organisational policies and procedures.
- Build and maintain effective partnerships with clinical services, community organisations, government agencies and other key stakeholders.
- Foster a positive, high-performing culture that values lived experience, continuous improvement, cultural safety and workforce development.
What you'll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
- Leadership experience in mental health, community services, disability, health or a related sector, with ability to lead, coach and develop a multidisciplinary team.
- Expertise in trauma-informed, recovery-oriented practice and evidence-based interventions.
- Experience in managing service performance, budgets, reporting requirements and stakeholder partnerships.
- Ability to provide proof of vaccination against vaccine preventable diseases, or willingness to obtain required vaccinations per Queensland Health partnership.
