About the Role

The role will be split between the Liver Transplant CNC (3 days) and Immunocompromised, hematology, oncology and transplant infectious disease CNC (IHOT CNC) (2 days) IHOT-ID.

In this role you will work closely with a Multidisciplinary team to support pre- and post-transplant patients. The Liver Transplant Clinical Nurse Consultant will work alongside the other Liver Transplant CNCs and nursing staff to provide case management and care to Liver transplant patients across NSW and ACT.

The applicant must be an Accredited Nurse Immuniser with familiarity and/or experience with immunocompromised patients, including but not limited to solid organ transplant (SOT) and hematologic stem cell transplant (HSCT), cellular therapies (chimeric antigen receptor T-cell therapy (CAR-T) and bi specific T cell engager (BiTE) therapy and patients with splenectomy. A familiarity with vaccination schedules and vaccine fridge management is desirable.

The Clinical Nurse Consultant (CNC) provides expert clinical advice to patients, carers and other health care professionals within a defined specialty through the delivery of specialist consultant service which:

• Develops implements and evaluates care management plans for patients with complex health needs.
• Provides leadership in the ongoing development of clinical practice.
• Initiates and utilises findings of research in the provision of clinical services.
• Contributes to the development and delivery of specialty related education programs.
• Participates in formal processes for the strategic and operational planning for the clinical service.
• A current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field.
• An approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.
• Leadership skills and excellent interpersonal, oral and written communication skills.
• The ability to work autonomously and in a multidisciplinary team environment.
• Extensive knowledge of good clinical practice, policies and procedure.
