BlueCare is seeking a Care Partner to deliver person-centred community care across Redland Bay to Victoria Point and surrounding areas. You'll oversee care planning, coordinate services, and work with clinical teams to achieve positive outcomes while travelling regularly to client homes.

You'll have a Certificate IV in Health or Community Service, a current Australian driver's licence, and access to a roadworthy vehicle. This full-time max-term role supports not-for-profit care delivery.
