Bolton Clarke is seeking a Full-Time Care Coordinator for our Caboolture Office to provide service coordination, assessment and care planning for non-clinical support programs. You will ensure timely documentation, model of care implementation and client-centred delivery.

Applicants should have a Certificate IV in Community Services or equivalent, strong MS Office skills and excellent communication. Police check within the last 3 years is required; fixed-term until May 2027.
