Luxe Care Pty Ltd is seeking a qualified and experienced Community Worker to join our team in a permanent full-time position. The Community Worker will assess clients' needs, develop appropriate support plans, coordinate referrals and help clients access community, health, welfare and social services.

You will work closely with clients, families, Care Managers, healthcare professionals, community organisations and government agencies to provide culturally responsive, person-centred support.
